I. Purpose: To recognize and honor one member of OAFCS per year who has made a significant contribution toward the overall effectiveness of the total organization and/or the profession.

II. Criteria for this award may include but not be limited to:

A. distinguished service such as significant achievement and/or outstanding performance of duty at the state level;

B. meritorious service given in time, talent and energy to the over-all program, to a specific project/goal(s) and/or to the effective operation and management of the organization;

C. achievement in extraordinary, distinct and/or different ways in the professional workforce and/or in community volunteer activities and/or to OAFCS;

D. consideration shall be given to members not eligible and not honored by a division and/or section awards.

III. Nomination may be by any member of OAFCS, divisions, sections, and districts.

A. Form Requirements

1. The form for nominating a member should include name, address, phone and fax number.

2. Present position.

3. Description of how this person has given distinguished service to the organization and/or to the profession.

4. Signature, address, phone, fax of person nominating.

B. OAFCS Awards Committee should request a recent resume from the nominee, if available.

IV. Selection: The OAFCS Award Committee shall make the final selection.  All nominations shall be kept by the Awards Committee for at least three years.
